Student Terms and Conditions
TERMS AND CONDITIONS FOR NEW STUDENTS
- The Doctor of Medicine Professional Study Program Registration Form can be downloaded from the website fkik.uin- malang.ac.id
- Proof of PMB (New Student Admission) registration in the form of a test card
- Bachelor's Degree in Medical Education or Bachelor's Graduation Decree with GPA minimum 2,75.
- Letter of Recommendation from Medical Education Study Program
- Certificate stating that you are not currently undergoing any ethical or academic sanctions
- Letter of Free from Administrative and Financial Liability.
- Certificate of Physical and Mental Health for the last 3 months from a government health institution.
- Drug-Free Certificate from a government health institution for at least 3 parameters in the last 3 months.
- Hepatitis B vaccination certificate or lab results showing anti-HBs results with a titer of at least 500 IU/ml (in the last 3 months).
- Test results Minnesota Multiphasic Personality Inventory (MMPI) in the last 3 months which MUST be done at Dr. Radjiman Wediodiningrat Lawang Mental Hospital.
- Scan your BPJS Card or other Health Insurance that is still valid and active.
- Scan a valid and active BPJS Employment Card
