  1. The Doctor of Medicine Professional Study Program Registration Form can be downloaded from the website fkik.uin- malang.ac.id<\/span><\/li>\n \t
  2. Proof of PMB (New Student Admission) registration in the form of a test card<\/span><\/li>\n \t
  3. Bachelor's Degree in Medical Education or Bachelor's Graduation Decree with GPA <\/span>minimum<\/b> 2,75.\u00a0<\/span><\/li>\n \t
  4. Letter of Recommendation from Medical Education Study Program<\/span><\/li>\n \t
  5. Certificate stating that you are not currently undergoing any ethical or academic sanctions<\/span><\/li>\n \t
  6. Letter of Free from Administrative and Financial Liability.\u00a0<\/span><\/li>\n \t
  7. Certificate of Physical and Mental Health for the last 3 months from a government health institution.\u00a0<\/span><\/li>\n \t
  8. Drug-Free Certificate from a government health institution for at least 3 parameters in the last 3 months.<\/span><\/li>\n \t
  9. Hepatitis B vaccination certificate or lab results showing anti-HBs results with a titer of at least 500 IU\/ml (in the last 3 months).\u00a0<\/span><\/li>\n \t
  10. Test results<\/span> Minnesota Multiphasic Personality Inventory<\/span><\/i> (MMPI) in the last 3 months which MUST be done at Dr. Radjiman Wediodiningrat Lawang Mental Hospital.\u00a0<\/span><\/li>\n \t
  11. Scan your BPJS Card or other Health Insurance that is still valid and active.\u00a0\u00a0<\/span><\/li>\n \t
